Are you looking to send copies of your records elsewhere ?

You must agree to the following to get to the request form

Request for records is only online (next step).
Your records belong to DOC EYE LLC. We must keep them for ethical and legal reasons.
But, the information on the records is yours so you are entitled to copies. This is standard practice for all professions.
There may be charges. Faxed copies are free. Printed copies are charged at 65 cents per page. This helps us offset the cost per page, postage, and employee time. We receive a lot of requests every week since people move a lot nowadays. Please review the statute below.
Turn around time is at least 3 days.
